NEW PATIENT REGISTRATION
To register at the surgery please bring along your medical card or alternatively there is a form at the surgery for you to complete.

ACCESS TO MEDICAL RECORDS
The surgery is registered under The Data Protection Act 1984.

According to the Access to Medical Records Act 1991 patients may view their records from 1991 onwards at the discretion of the Doctors.

A request must be put in writing to the Practice Manager and there is a fee of £10.00 for this service.

FOREIGN TRAVEL
If you are planning to travel abroad and need advice as to which injections are necessary, please contact reception in the first instance .

NON-NHS SERVICES
There is a charge for non-NHS work carried out by the Doctors for form completion etc, please ask at reception for full details.

COMMENTS, COMPLAINTS OR SUGGESTIONS
We welcome comments, complaints or suggestions at any time, where possible in writing, addressed to the Practice Manager.

All complaints will be dealt with as soon as possible.

If you are unhappy about your treatment in surgery, don't tell others tell us!
